Dress Code: No halter tops or swimsuits permitted.

Snacks: Your child will be provided a variety of snacks and drinks each evening. If your child has any allergies, please be sure they are listed above.

Medication: Please note that VBS staff and workers of Lighthouse Baptist Church do not administer medications of any kind to any child.

I hereby give permission for my child to participate in all activities associated with Vacation Bible School at Lighthouse Baptist Church. In the event of an accident or illness, I authorize church personnel to obtain emergency medical treatment for my child if I cannot be reached. I understand that reasonable precautions will be taken to ensure the safety of all participants, and I agree not to hold Lighthouse Baptist Church, its staff, or volunteers liable for injuries or illnesses that may occur during this VBS event. I also acknowledge that photos and videos will be taken during this VBS event, and give my permission for photographs or video recordings of my child to be used by

Lighthouse Baptist Church for ministry, promotional, and informational purposes, including print and online media.
